In November 2015 Premera and Landmark Health teamed up to provide in-home medical care for patients with chronic, complex conditions in the King, Pierce, and Snohomish counties of Washington state.
In 2017, Premera and Landmark Health will expand that care to eligible members in Spokane and Thurston counties.
Members will have the option for:
- 24/7 care from Landmark providers in their own home
- Scheduled, ongoing visits based on the patient’s needs
- Access to Landmark pharmacists, dietitians, nurses, and social workers
- Direct communication with primary care providers to collaborate on care received from Landmark sources
