Inbound Health and SSM Health have partnered up to offer post-hospital in-home health care services to patients. The SSM Health at Home program – Recovery Care at Home – will apply to certain patients who are ready to be discharged but who still need services traditionally offered in a skilled nursing facility. Phase one of the partnership kicked off at SSM Health St. Mary’s Hospital – Madison in April 2024.
Dr. James Lehman, SSM Health Wisconsin Regional Clinical Program Medical Director commented on the health benefits of the new program: “Research shows that patients prefer to receive in-home care and often recover faster within a familiar environment and are less likely to return to a hospital,” he said. “Patients being cared for at home spend less time in bed and are able to be more mobile so they can continue their daily lives with their families, friends and pets to achieve better health outcomes.”
The new partnership will offer patients the following:
- Personalized, daily in-home and virtual visits from specially trained health care providers
- Digital tablets to provide enhanced remote monitoring and 24/7 assistance
- Arrangement of care visits by a care coordinator
- Home infusion services, lab and imaging services, and medical equipment
